Aim Cyst-like lesions in the mandible rarely develop into malignancies, and reported incidence is between 0.3% 2%. The present study describes a rare case of primary intraosseous squamous cell carcinoma arising from an odontogenic cyst. Materials Methods An 80-year-old male was referred to Trieste University Maggiore Hospital (Trieste, Italy), with acute pain left retromolar area. initial examination revealed extra oral swelling without paresthesia IAN. Following intraoral examination, mucosa edematous, percussion experienced on lower second molar. Panoramic radiography retained wisdom tooth irregular radiolucent area molar mandibular angle clear margins. Computed tomography diffuse bone resorption extensive loss cortical lingual side. Results A biopsy performed during surgery remove tooth, pathological diagnosis epithelial lining MRI contrast agent also performed. Shortly after patient has been proposed complete resective mass including infiltrated areas then reconstructive part restore function aesthetic. Conclusion follow-up large inflammatory cyst essential for early malignant neoplasm. This approach allows invasive treatments major survival these patients.
